Needing help with making an automated word document using a windows form (VB)

I have a minutes document in word. I am currently creating a form with Visual Studio (VB) that inputs the data unto the word template. There is a section where the user will keep adding to the document by clicking a next button until the user is finished. There is no way for me to know how many items the user will need to add (clicking e next button). Right now I'm using panels to to separate what the user sees based on his/her actions. What is the best way to keep the panel showing up (on next btn click) until the user clicks the checkbox stating that this is their last instance? I have wrapped my head around this, and keep hitting a dead end. The trick is that in the word document, each instance will need to be numbers, without erasing the previous data, and the user will need to be able to click the back button to edit previous entries if need be.

Thank you for all your help!

Josh
